Bolus calculator wizards in insulin pumps may help fine-tune and improve glycemic control
Intensive insulin therapy using a continuous subcutaneous insulin infusion pump is sometimes necessary for patients with type 1 diabetes. Insulin pumps come with different optional features that may measure different parameters. The importance of these features and the parameters they measure in effective glycemic control was assessed in a study of 88 consecutive type 1 diabetes patients on CSII therapy. Patient data collected included demographics, anthropometrics, disease characteristics , glycated hemoglobin , blood glucose, carbohydrate intake, insulin dosage and pump adherence. Pump features assessed included the manual prime and rewind function, the bolus calculator wizard and the number of glucose readings. The use of the bolus calculator, which is an optional feature of many pumps, was significantly correlated to positive glycemic control. Patients who used the bolus calculator feature in 50% of their boluses reported significant reductions in HbA1c and glucose values . This correlation remained after adjustment confounding factors. The results suggest that the bolus calculator feature in an insulin pump may be a useful tool in achieving glycemic control and this is attributed to the fine-tuning of bolus insulin dosing using the bolus calculator wizard. In order to confirm these findings, larger prospective studies are warranted : 69).
